(b) Primary 5 & 6 School Transcripts (Original and a copy, copy only if applied via post). (c) Birth Certificate or Hong Kong I.D. Card (Original and a copy, copy only if applied via post). (d) A Student Photo. (e) EDB Application Form for Form One Discretionary Place. (f) Two Envelopes, Three envelopes if applied via post (With stamp and the student’s parents name clearly).
